Before You Begin
Why we ask for this information
This form helps me understand your situation before we meet so we can use our time together as effectively as possible. It also sets out the nature of our professional relationship and what you can expect from working with FORGED.
There are no right or wrong answers. Answer as honestly as you can. The more I know before we start, the better I can help.
Confidentiality — what this means in practice
Everything you share is completely confidential. Your information will never be shared with your employer, partner, family, or anyone else without your explicit written consent.
There are three exceptions where I'm legally required to act, regardless of confidentiality:
- If you disclose a serious and imminent risk of harm to yourself
- If you disclose a serious and imminent risk of harm to another person
- If there is a reasonable belief that a child is at risk of abuse or neglect
In any of these situations I will always endeavour to discuss this with you before taking action where it is safe to do so
The professional relationship
FORGED Men’s Coaching & Counselling provides counselling and coaching services. As a registered ACA counsellor and certified men’s coach, I am bound by the ACA Code of Ethics, which includes obligations around confidentiality, professional boundaries, competence, and client welfare.
This is a professional therapeutic and coaching relationship. It is not a friendship, and it is not a medical or psychiatric service. If at any point I believe you need a level of support beyond what I can appropriately provide, I will tell you honestly and assist you with referrals.
